First off, I cannot speak for Karen or MJW's review.. only they can do that. However, I feel MJW should not have mentioned the "without condom" part, because I consider that a taboo subject and I'm sure it's not something Karen would want published. MJW should feel guilty about posting that. If I were Karen, I would ask for that review to be removed.
The above being said, I have to say that it indeed happens sometimes. I personally have experienced no condom FS for brief periods, usually right after foreplay, with two providers. One provider is reviewed on TER, the other is not. It usually starts by rubbing your cock on the outside of her pussy and on her clit for awhile, which feels great by the way. Then, in the heat of passion, it can happen. In both of my experiences, it was the second or third appointment, NOT THE FIRST. So, we knew each other fairly well, and trusted each other. I have had an experience similar to the previous post but not with Karen. It happened just like the previous poster described - genital to genital rubbing initiated it and after a while penetration, but not to completion. I find it amazing that people are amazed this happens. Just think about all the unplanned pregnancies and constant spread of STD's.
