If you've done all that and still want more, you won't find much else outside of being lost in Thailand/Indonesia for awhile.

The thing to do is just have loose plans for when you get there, a budget, and a start/end point for coming home. Then, hire a guide to adventure with. They'll be able to translate, show you all the cool local stuff, and generally make sure nothing bad happens to you.

to suggest a double like many 40-year old guys would do.  Getting into bed with two hot ladies proves 40 is just a number.  Since you're hot, too, you might not have to pay if you tell them its your birthday.  Me saying it didn't work.  Lol

 
If you want to go somewhere, I like the Caribbean in the spring.  Mild weather, ocean water relatively warm compared to other places.  My specific suggestions would vary depending on what you like to do for fun.  St; Barts for upscale relaxing colonial vibe, Mahogany Run on St. Thomas for golfing,  Martinique for a touch of France on this side of the Atlantic.  Atlantis in the Bahamas for water fun and working on your tan lines.
