When I've been asked to come visit a guy in another city/country..he had to pay for my fee, my hotel, my airfare...a few times the guy paid for my airfare/hotel with a credit card then paid my fee in cash...where as when I traveled out of the country they just sent me airfare/hotel/fee all at once and I was required to book the flight and hotel....your best bet is to ask the lady in question and see what she feels comfortable with.

for out-of-area trips. This is something you and the lady will have to hammer out beforehand. As to hotel location, same thing: work it out before she leaves for your city.

I agree with above.  Usually the price you and the provider agree on is just her fee.

The only time I booked with someone who's real name I didn't know, she bought the ticket and I reimbursed her.  All of the other times, I shopped the rate and bought the ticket in the providers name.  I guess it would depend on how comfortable she is with you on how she would want to work that.  If I wasn't picking her up at the airport, I would expect to pay her way to/from on my end.

Depending on how far she is travelling and how long she is going to be in town, you might not need a hotel room if you can use your own place if she is agreeable to that.
I don't know regarding Philly flights but it shouldn't be too hard to get in/out of DC via Reagan or even Dulles.

If you get her a hotel, I would expect you would book it and  for pay for the first night anyway.  You know the city better in terms of convenience for you unless she has a favorite hotel you don't object to.
