From reading articles about Shayla, August and Olivia, mental issues and possible drug use may have been huge factors. Not to cast a wide assumption, but there has always been indications that drug use amongst starlets is wide spread. Unless they implement drug screening I am not sure what can be done.
Perhaps mental evaluations, drug screening, etc., but past that I'm not sure what the answer is.
What happened isn't clear (relapse? where did she get the drugs and / or booze?) but it says that she was in a residential rehab place. Poor patient analysis? Poor security? Poor supervision?

Multiple posters declared, "Something has to be done." There is a fairly general protocol to follow after any FAILURE. Before you can decide what "something to be done" is, you have to figure out what happened and what went wrong. The news report said that she was in residential rehab, which SHOULD be a safe place. So what went wrong? Doctors misdiagnosed her? ("She's good. No need to watch her.") Security? ("Neighborhood drug dealers have keys to the main entrance and ID cards.") Poor supervision? ("Nurse Ratched was supposed to be checking on her every hour, but didn't.") . Friends and relatives of others at that facility will surely want answers to those questions and many others. And then they will want "something to be done."
