In order:

Either PSE or GFE, whichever she is more comfortable with.
Prefer BBFS (both parties with recent clean tests) but CFS is fine.
I try to always have pictures or video as a memento to look back on fondly.
Two to three hour meetings are my personal preference.
I prefer sub-$1000 but have gone as high as $1350/hour (averaged out for a two hour meeting).
Independent.  Never used an agency due to the increased costs.

1) I have a pronounced oral fixation so while I am pretty flexible about the rest of our activities, if BBBJ/CIM is not part of the program, I'm not booking it.
2) I'm not one of those who HAS to have BBFS and indeed have only done it once.  I also totally respect the girls who won't do it, regardless of what anyone says about recent tests, I can appreciate not wanting to take the risk, however small it might be.
3) I have done both photos and video on a number of occasions but have also found that it can detract from the experience.  My custom now is that when hooking up with a girl for the first time, especially if it's some PS I'm really into, I'll maybe take some pics or video at the beginning but then put the phone down so I can be totally in the moment.
4) Depends on the girl but usually an hour.
5) I'm cheap as hell so if you know where to look, you can find some pretty well-known porn chix for as little as 200/300/hr.  My upper limit is probably a grand, maybe more, again depending on the girl. I know it bugs some people here when I write this but this "hobby" is currently most definitely a "buyer's market."   Accordingly, there are a number of girls who are doing amazing things with great bodies and beautiful faces for less than some might think.
6) Definitely prefer indies, have only gone through an agency on a couple of occasions when it was someone I really wanted to see (Alana Evans leaps to mind, so glad I booked her when I did since she was escorting for about a half an hour or so!)

The only way to go is indie!  It's best to deal with the provider directly.

1.  PSE.  The goal is to live out the fantasy you see on the screen.  I am inclined not to book if CIM, COF, or BBBJ is not offered.

2.  CFS.  BBFS is still too risky.

3.  If the price is right I'd like to photograph and or film it as I've done in the past.  It is a great momento.

4.  If funds permit, I like to shoot for 2 hours, but I've done 1 hour and 90 minutes before.

5. Anywhere from $500 - $1000.

6.  I prefer Indie due to rates.  I wouldn't rule out an agency if I reaaalllly wanted to see a girl but due to higher rates, I would definitely research before pulling the trigger, and it may only be an hour.

Video/Photo is hot but it really gets in the way, so I have mixed feelings about it. It's one of those things where I might go for it once but then when I realize how cumbersome and repetitive it becomes I stop after a bit.  Maybe a go-pro head-mount cam might be better? lol  

Also, I have had a girl do the video editing for me and I felt that was a really nice service. I'm paranoid about privacy though I'd only allow a girl have a copy to edit if my face isn't in the video (which is the case only for POV type of videos).  

If I'm seeing a girl for the first time, I book 1 hr.  If she was great, I may go for 2~3 next time whichever comes out to be the best deal. Rarely ever do overnights though. Not my thing.

I've seen anywhere from 250 to 3000. 3k was my maximum ever paid for a UTR. In general, the ones I repeat are under or equal to 1k.  The ones I see much more frequently are less than 600.  

I use both indies and agencies, so no preference.

I would try to contact a lady directly via social media. I would stay away from agencies as they hold personal information and I would not trust right now. Also if you guys can hold it in your pants I would avoid hiring anyone until prices come down. No one should pay more than $1500 per hour. the prices of $1800 and above is something we can control by not hiring right now.
