Now that more information is out, it appears that investigators have concluded that there were two causes of the collision, but the main one is that the tower was being staffed with only 19 ATC's when the operating protocol calls for 30, so they are eleven ATC's short, and that is why one ATC was handed both the Commercial Jet AND the helicopter, which is NEVER supposed to happen. The reason is that these two aircraft are on a different frequency and can't hear the other one talking to the tower. Military uses a UHF band which the commercial pilots are not allowed to access. When you have just seconds to avoid a collision, switching bands and repeating what the other aircraft said for the other pilot, they are already collided.
The other error was that when the helicopter was asked if they had a visual on the jet, and answered affirmatively, he was looking at the wrong aircraft, one in the distance at 12 o'clock and not the one coming from the 3 o'clock position.
The ultimate responsibility for the understaffing, which created the situation that caused the collision belongs to Mayor Pete, who was Secretary of Transportation under Biden. The memos allowing the airport to continue to operate at full capacity while the tower was understaffed where originated in the DOT, which oversees the FAA. You remember Mayor Pete, right? He's the guy who got the appointment as Secretary from Biden when his only experience was that he preferred to ride a bicycle rather than in a car. Way to go, Mayor Pete.
