probably din't go far or fast enough to arm the warhead. Most have some sort of "setback" device to minimize the risk of in-bore detonation - ie., the projo has to have enough acceleration to force some sort of safety open to arm the fuze.

No telling what went wrong, except that it looked like it went off when it hit the firing pin. My guess would be that it was Russian or Chinese design and 3rd world manufacturing quality control, and he just got the lucky fuze. Or maybe something happened that the warhead wasn't sufficiently separated from the propellant - like a crack in the shell, but either way, I'd bet it was a mfg defect.
Whatever it was, it was enough to turn that tube into razor blades. He probably never felt anything.
