Sorry about not making the last announced trip gentlemen...something came up!

However, I will be in Scottsdale (for sure this time as I already booked my hotel) 12/22 - 12/24.  

I am only available for a limited number of appointments so don't wait until I arrive to contact me.  Best thing is to send in my screening form as soon as possible. This does not obligate you to an appointment with me, it just increases the chance for a last minute appointment once I arrive.
