Has any one had any luck finding woman who provide private lap dances  that turn into full service?

Ok maybe I should have been more specific.  What providers have you seen that provide this service?  Witch ones where realy good at it ?

Good question.  There's a huge difference in lap dance quality from a lady who's actually performed them to make money and someone who maybe wiggled on a couple laps know that she's already made her money.  The first is much more erotic, sensual and arousing.  Can't help you since I haven't been to PHX in years, but my recommendation: email providers and ask if they can.  Trial and error will find you the right girl.

OK I will try one more time.  Is any one willing to give me the name or add site for some one they have enjoyed.  I would simply use the search review function on TER, but this is not something that is asked about (we are asked to rate massages, but not dance).
