What are all the possibilities with regard to why a provider has reviews, and then one day is gone?  There is a provider who is currently advertising who say she is "reviewed" and even posts links on her website to dead TER pages.

I know the reviews were there at one time.

What are all the reasons they would be down?  Just can't remember.

How about she is delisted/banned from TER for one reason.

... but that's the basis of the question and answer.  I think there are multiple reasons for being banned or delisted, and I think a provider can request the removal, but I can't remember the circumstances, and... I think there are other reasons or ways that some just vanish.
What are all the various ways that someone would have a "rating" and then just be "gone."

If you click on the button you see a number of reasons for any one to be banned.  Hope that answers your questions, if not may I suggest asking Dingus this in a PM.

if a provider is banned/delisted by the ter gods or by her request, then you get an error message that says 'this provider's profile is no longer available'

if you are searching and the search comes up empty, then she has never been reviewed.

if you are getting some sort of blank page, it might be your puter.

i'm just not sure what you mean by gone.

I have been told by two providers who at one time were banned from TER that it was because they let a hobbyist use their computer to write a review on them.

They told me that the hobbyists had posted  reviews on other providers, but the TER system still flagged the review as coming from the same computer that the providers used. That was enough for the provider to be removed from TER as the system saw it as a self-review. Eventually the provider was able to get back on TER.

Lesson: Never let anyone post on TER from your computer except yourself.
