I am new to the hobby, still deciding on my first provider. Today I got my annual full health test done and I'm waiting to hear back for results. What is the proper etiquette between hobbyists and providers when disclosing STI or STDs? In your experience, is this information even discussed?

in my experience, it's rarely discussed.  
i havn't had anyone ever ask before.
it's one of those things where everyone knows there's risks,
so practicing safety is always good. we only get one body after all- treat it well.
though, if a client ever would like to see such results, i personally wouldn't have a problem
handing them over.

That's helpful to know that I could always ask. I don't necessarily want it to be a mood killer, however it could definitely quiet any fears beforehand.

You can ask and you will get an answer.  But the problem is that you don't really ever know if the answer you get is true.  

Look at it this way ... how long is it going to take to get your test results back?  A day?  A couple of days or even weeks?  Only you know for sure what kind of activities you have been up to since having the test done.   Do you see what the issue is?

And you see a provider or a hobbyist anyway without disclosure IMHO that is not OK. Yes, there is risk in doing this. But one of those risks shouldn't have to be that someone is intentionally and knowingly infecting you with something and not telling you.  

In my personal opinion there is a general expectation and assumption of D&D free P4P.

If you saw a provider who had some kind of nasty STD, do you really think she will tell you that she has it?  You would be running for the door.  At least I would.

After all, nothing kills the mood better than starting off a session by saying "Hello, my name is Moneypenny, I have herpes, gonorrhea, chlamydia, crabs, and some kind of nasty rash that they haven't yet identified, but they believe that if you catch it, it might make your dick fall off.  Whew!  Now that we have those details out of the way, why don't you come here and give it to me up the ass!"

By the way, if you were to tell a provider these sorts of things, you would also get your butt thrown out of her incall.  

Is it any wonder why nobody discusses these things?  You would only hear what you want to hear anyway, whether or not it is even true.  You already know the risks, so protect yourself then take a shower and use soap.
