I'm asking because there is a fairly high profile Domme here in phoenix with no reviews. Hard to understand why.

Generally if a provider wants her info removed she can't still be actively advertising anywhere... The provider needs to be either at risk of exposure, LE, family or retiring to my knowledge. If they find ads of the person de-listed they will re-list the review page and likely not remove it again.
