I believe the provider you are referring to is Indy Lacy, whose former web site was indylacy.com.  Her website is down and she does not appear to be working, at this time.  There are several local agencies that are reliable, the best of which is Hot Friends.  Hoosier Hotties also has a good reputation, but for some reason, all reviews of providers from that agency have been removed from TER.  Be careful of other agencies in town, though, as most are of the cash and dash variety, or try to squeeze you for extra money.
