I'm not sure that there is a difference. Obviously there's going to be some divergence between, say, a "crack whore" and an "elite courtesan," but I'd say that both (and everything in between) fall under the umbrella term "hooker."

It'd probably be easier to analyze the differences between low-end, mid-range, and high-end providers. The interchangeable terms we use don't really mean much in and of themselves as far as I'm concerned.
