TER

I attempted to submit a review, but was unable because provider does not want her e mail add out there.  I see many provider reviews that state e mail  NONE.  So whats up with that???

You don't need an email address. There is an asterisk next to email address on the form

An asterisk means that information is NOT required.
However the provider has to have some means of contact. If there is no email address there must be a phone number.

in addition to email or phone...

... a pager number or reservation form is acceptable also.  Not sure if anyone still uses a pager, though!

You must be able to give some way on contacting her.

It can be a phone number or an email address, either one is OK.  As long as one or the other is provided, your review should be accepted.

And I knew that but left it out on purpose because I know nobody uses a pager anymore. Lol. :)

OK, looks like someone needs to provide the actual rule on this since...

... some of us keep getting it wrong, so here it is.  Note that there's FOUR forms of communication that are acceptable for a review:

"The lady being reviewed must have an advertisement on the web that includes her name and any one type of the following contact information: phone number, pager, email address or reservation form."
