First, make sure she is okay with reviews. Some ladies do not want reviews.

Her TER ID will be created once you submit a review.
Enter all the relevant information.

...because, as the first reviewer, the information will be "written in stone" and will be difficult to change if you've made a mistake.  

Don't guess about any of the criteria.  If you didn't do anal and the subject didn't come up, you should put down "Don't Know."  It's better than guessing.

Just in case you're really, really new - click "Account Manager", then "Reviews", then Submit a Review."

I have seen TER pull the reviews of ladies who are reviewed by brand new guys with no review history. This is to prevent incidents of self-reviewing i.e. the lady reviewing herself using a different ID.

While I don't know the exact TER policy surrounding this, I just told you what I have observed in the past. The policy could have changed now, from last year when I observed it, so take it for what its worth.

If I were you, I would shoot a message to TER using the "Contact US"  option, and ask them if it would be OK for you to review her. This would save you some time and heartache for sure....

If she has a website and either a phone number or email address, you can review her.

your review will create her profile.  A comment about contact, she must either have a personal website or an ad that can be linked to, or she cannot be reviewed.  Contact can either be by phone or email.  If you know her phone number, ask if she wants it on the review or not.  I just did a review for a provider who did not want her phone number posted, and since you have to enter a number, I just used 555-555-1212.  Some gals just want email contact and will give you a phone number after screening is complete and date is set, so you can get hotel room number or address to her incall.
