I would look into using something like p411 - it is much, much easier than asking for references. The agency will first verify you (fairly simple process). Once verified, if you see a p411 provider, you can send her a request through p411 for an "OK". I believe this is just email to her asking her to OK you, and she can just click a link to respond. As you see more providers, you get more OK's. Also, within p411 you can see providers in your area who are members, and under their profiles, it will list - willing to see newbie, willing to see plus 3 members only (meaning someone with 3 or more OK's).
I think there are other similar services, but this is the one I have used for several years. On one hand it is very easy for you and providers to use - she is not getting calls whenever you need a reference, but on the other hand, these services cost money, so if you are not planning to hobby that frequently, this may be a costly venture. I am not a frequent player, so I think I do lose money on this service, but when I do want to play, it is much, much easier to see someone new.

You need to tell the provider who is asking the reference enough details so that the provider giving the reference can remember you. For instance I tell when and where I saw her. And as I'm foreigner it's easier for them to remember. Also I might give some detail about our meeting that she might remember. I'm not sure but they might also use your first name and number. I know that some providers know me from my number that I'm ok and don't need to be screened.
So even when rebooking a provider you've seen don't expect her to remember you.
