To answer your first question, typically the lady has secured a room for herself to service as a incall location. As a side note: you probably wouldn't want the room in your name, or to leave someone behind to use the room after you left. Additional charges, damages etc could be added to the bill you will receive. Even though there is only a slight chance of this, you still need to protect yourself.
Second question: It probably would be a good idea to give yourself a little trim a least. The old twig and berries will get more attention from the lady if everything is a little more tidy.
Make sure you read the newbie manual, and read up on all the terms and definitions. Formulate questions not answered there, and come here for help. You may want to join one of the screening services like RS2k or whatever is used in your area. Check your local board.

2. Guys who shave think everybody should. Guys who don't think it's no big deal. I happen to be one who doesn't so my 2 cents is this; I have been seeing providers for over twenty years. No woman is going to turn you away as long as you are clean, polite, timely and respectful.
I shave and am completely shaved. I don't have body hair so it looks ok. If you are like a chimp, you may want to trim down, and have it blend with the rest of you. Finally it makes alot of difference if your in shape. Your waist needs to be half your height. 6 ft tall you need to have a minimum of a 36 inch waist. When you talk about a six pack and you mean beer, chances you need to be working out. I look at the hobby as a reason to workout and look my best, without clothes.....Just me....

The amount of risk is proportional to the amount of care you invest in your selection process. Stick with well reviewed ladies from TER. Looking on Eros, Cityvibe, Back Page or even Craig's List (proceed with extreme caution where CL is concerned) is a start when it comes to searching for escorts but unless you can find the lady well reviewed here on TER you are always taking a chance.

I don't see more than one client a day and I offer in calls with a request for a 2 hour min to help offset the cost of the room. I do it because I understand he may not want the charge to appear on his CC or doesn't want to pay full price for a room he is only going to need for a short amount of time. This way we are kind of splitting the cost.
As far as shaving goes... I keep mine nice and trim (not bald), it would be nice if you did too.

Ladies that offer incall at hotels do not charge hotel fees.
But I have traveled to see someone in particular and he arranged for the hotel for us.
Most gentlemen that I meet are not shavers. This is mostly because of SO's, I think. But some do shave.
As always I love trimmed and manicured gentlemen, but I understand each situation is different.

Your questions are really good, let me try to give you some real honest feedback.
1) Hotel room. if the lady is meeting only you (like an incall) and books a room, then she would either hope you will pay for the room in full, or at least split it with her.
As for your other 2 questions, this is really a matter of "preference". Since most ladies here will not want to say anything to cause them to lose business most will say it doesn't matter.
Do you see - for some, the fear of letting our preferences out might dissuade someone from booking with us.
So please bear this in mind.
2) shaved / natural / trimming. if its not too much trouble, a trim, would be nice, just prune back the shrubs a little. My hubby trims down to about a 1/4 inch. Ive dates shavers, and wow, thats really nice, but not a "requirement". Cleanliness on the other hand is a REQUIREMENT.
3) cut vs uncut. this is purely a preference issue that for most of the providers we know, is in no way a requirement. I happen to enjoy a cut man a little more, but, im a professional and an uncut man with me would never know the difference since its just not that big a deal for me.
