Is there some place I could read about donation etiquette?

I originally assumed that donations were given directly to the provider, in lose bills, before services were provided.

But now that I've been reading posts and reviews, I'm hearing about "the envelope," and different places "the envelope" is left (ie. dresser, instead of being handed directly to the provider), and different times "the envelope" is produced (ie. when she goes to use the restroom).

Is there some ritual that I need to know about?  Help.

Handing the donationaly openly to the provider before activities is usually low level Street Walker(SW)protocol. Many internet providers will instruct on their particular favored etiquette in the text on their website. Generally speaking; the less you make it look like a transaction the better.

Some providers will let you know how they prefer to receive the donation.  If they go through the trouble to specify, follow their instructions exactly.

In the absence of instructions, present the donation as discreetly as possible.  Always use an unsealed envelope (or a card in an envelope - that's a nicer touch).  If I have no other instructions, I usually take the envelope, my cell phone and my keys and place them on a table or counter in the room.  Then as I leave, I take only my keys and cell phone.

Or, if I think the provider needs to verify the money, I'll leave the envelope more obviously away from my stuff, then excuse myself to the restroom to give her time to count it if she needs to.

If you possibly can, avoid handing the envelope to the provider directly.  She may not want you to - and it's always a good idea to be as discreet as possible about the donation process.

. . . request that we put the envelope in the bathroom, so that they may count it before the session starts. I have not seen one that requested that yet.

I normally empty my pocket of keys and my cell phone. The I turn off the cell phone and place it next to the envelope. It would look like the envelope is mine; but I'm so forgetful :-( . . . I always leave it behind. :-)

. . . I would think leaving the donation in the bathroom is a good idea. The ladies most likely use the bathroom before a session.

place the envolope in view of the provider so she can see it. DO NOT HAND IT TO HER DIRECTLY if you do she may think that you are le also she maybe le and you will have a hard time trying to explain yourself. a very good provider will see the envolope and sometimes excuse herself to count and sometimes she will trust you.
