I seldom partake of the entire menu, particularly if the menu includes things I don't like. There are some things that are on some menus that I won't do because I believe they are degrading. Maybe I'm just a prude, but -- hey, I'm paying for the time and I can be a prude if I want.

But I'd never dream of asking for a discount for that. My attitude is just like the disclaimer: I am paying for the lady's TIME, and anything else that may develop is incidental.

There are just too many ladies out there who have so much on the ball that it would be a real insult to try to discount them because you didn't use the full menu.

I disagree about stating your desires tactfully before the appointment. Since it is illegal to offer sexual acts for money, many ladies will be very wary if you do this and may screen you out.

The urologist who did mine used a scalpel with a  #11 blade. I think that was fairly common then, but more and more they're using a minimally invasive technique called keyhole entry, where they poke through the scrotum with a pair of closed forceps, which are then used to minutely dissect the membranes of the spermatic cord and grasp the vas deferens so it can be pulled outside, transected and ligated. They use a local anesthetic, usually lido, so the procedure isn't painful, but the post-procedural pain can be excruciating, although nothing compared to childbirth I wouldn't imagine.

Brave??? Puhleeze! Having kids is definitely a more painful experience. Especially with someone you don't really like all that much.
