Usually each gets normal rate but alot of times if one is higher they will most likely tell you the rate they agreed on in total.

 
Just ask to verify if your concerned or unsure and pay  it all in the envelope as usual....

depends of the lady I am doing duos with we have a different rate. Some wants their own rate, some agree to a flat rate divided by two.  
Some ladies have an special rate because it's fun to be accompanied by another lady....
