1. Press the "Provider Reviews" black button near the top of
the page.

2. Click on the "Search reviews" button on the left hand menu.

3. This will bring you to the search form.  You can leave it
as is except set the location to blank (top of menu) and type
in the phone number.  This should bring up a page with the
search results.

Note: for old profiles, you may want to set the Exclude bad
web sites to "No".  It defaults to "Yes".
