Make it easy to see you, not hard. You already got him hard in another way that he prefers!
Having too many questions on the form can make you sound way too picky and high maintenance... too much like his SO! And too many questions can make guys really nervous.
Her advice her is right on. My "form" has like two questions. I prefer to ask screening questions myself and use that opportunity to start getting to know each other and creating a banter. And I've mainly done that because so many guys say they don't like forms at all because they seem too impersonal.

I give clients the option to send me their screening info via email, but more than half choose to use my form anyway. Despite what some have said, it appears that the web form is actually preferred by most clients who see me.
Now, about your form. I have a few comments/ideas that may help:
- First, the form should be on your contact page, not on a seperate "screening" page. It's the last link on your site. Most have already sent a message from your "contact" page before they even read the screening page.
- Don't ask about a client's relationship status. Reminding a man about the wife he's about to cheat on isn't the best way to encourage their business.
- I don't see any reason to ask for a client's education level.
- When you ask "City?" Are you asking his city of residence or what city he'd like to meet you in?
- Don't ask about race unless you exclude certain races from visiting with you. If you see all races, having the question on your screening form makes if appear that race is a factor in your decision making.
- Most clients aren't going to send you a photo. I'd remove that too.
- Not sure why you're asking if the client is a traveler? If you are seeing him outside his city of residence, he's obviously travelling.
- Asking about best times to call and text might make clients nervous. Usually clients call ladies when it's safe for them to do so. No one wants a questionable missed call, text, or voicemail on their phone because they couldn't answer right when you called.
- "Preferred location" is really vague. Do you mean Incall/outcall? Or do you want his address if outcall? Hotel name? Do you want him to say "your place/my place" What do you want typed here?
