Has anyone heard of a provider asking you fillout infomation on there webistye and pay $1.00 via credit card for them to verify ?  IS this real or a scam?

Dont do it! Not worth the risk! Even if she is legit, which is incredibly unlikely, its a very unprofessional thing to ask of you.
