There have been a couple of providers who seem to have simply disappeared from the pages of this website. . . How does this happen? Can they personally request that they not be reviewed here?

One possibility is that they requested their profiles removed.  Another may be that the providers somehow infuriated the TER gods and were removed for some infraction of the rules governing the site.  This happened to one of my ATFs.
