just fill out the form with her info ... name, phone number, website, etc  and the people who review reviews usually match it up just fine. And if they don't  just file a problem report after it is posted explaining that the old profile and the new profile are the same provider.  They will have to have some info in common such as a phone number, e-mail address, or website or they may not match it for you.
