Someone created a profile for me even though I asked him not to.  I don't want ANY publicity.  I want the profile removed.  The profile also includes my phone number which causes me huge security problems in my real life.  I've emailed admin and used the "report a problem" button on my profile to no avail.  Is there any way to get it (profile) removed?  I would prefer not to participate in TER, or any review site.

if you have ads and/or a website up anywhere they may not comply.  Delisted providers are generally considered inactive, so if you can show that you're not actively soliciting appointments, your odds of getting your review removed are better.  

Also, they are more likely to delist you if you have safety or security reasons to do so, but in that case you wouldn't (or shouldn't) have any ads up.

If, however, you do have ads up and/or you have a website, but you just prefer no reviews, then you are probably SOL.  

If you can prove that the review is of you, you CAN ask them to remove your phone number and show as 'email only.

so long as your review is not a rip-off.  You may need to prove you are the person who is reviewed.  
You really should not use your real phone or email...  even without reviews...  but still if you are concerned about being 'outted' then you should be able to be 'voluntarily delisted'.  That means you can not be reviewed in the future, profiles taken down, and can not be discussed on any board.  If you are V Delisted, you could get it reversed in the future should you change your mind...  though that first review will be restored.  
I'd suggest you repeat what you've done & give the admin someway to verify that you are the person.  
Having ads up is required to be TER reviewed but it's not totally involuntary.  A provider can choose to be UTR or have a no review policy.  One can not 'pick & choose' only good reviews however.

She should nit be SOL.
A provider can be active and request no reviews. Maybe she just needs to insert somewhere on her website that she has a no review policy. In TER's info on delisting (in the self help center) this is the first sentence:

"If a provider has a no review policy in any form she is automatically de-listed.

in the past it is true that a no review policy would get you delisted.  That has apparently changed, however, and just like with other recent rule changes, the self help manual has not been updated.

Most recently I've heard more than 1 girl say that they were told that it is fine if they have a no review policy, but if they have ads or a website, and if a review is then posted, that the review will be allowed to stay up.

...from your profile.  I've seen ads which have phone numbers but TER refused to include them in the profiles saying the providers did not want them included.  There are also profiles which have the non-functioning phone number "555-1212."

.. and not a burn phone.  TER removed my burn phone number from my profile when I asked, without issue.

It is not my usual cell number but it can still cause problems.

In answer to the suggestions here, I have no ads and no website.  I deleted them when the review went up. My website DID have a firm statement about no reviews, and here we are.

I'd suggest you use the "Report a post" button under your first post on this thread.  Support got back to me within a day.  In the report box, tell them it is you, that you want to be voluntarily delisted for personal & security reasons.  Ask them what you need to do to get the profile removed.  I don't know what proof they might require.  
Quote your 'Stage Name' as given in the Profile & the TER number assigned to you.  Provide a Link to your profile so they get the right one.    
I always do these things when filing problem reports.

I used the "report a post" button and was told that I couldn't have my profile removed because I have ads.  But here are no ads.  None.  Unbelievable.

I've heard the same thing from dozens of girls trying to get delisted. They're being told that they can't have any ads or a website anywhere before TER will remove their profile. The problem with this is that there are tons of sites out there that simply copy/paste ads from other websites and post them to their own. Then we have no control over them and they often don't respond to requests to have our info removed.
