I'm interested in going to a massage parlor in San Francisco, but don't really know the protocol. I know you pay an entrance fee, but do you tip the girl right away or do you start the massage and try to discuss extras? Does anybody have any suggestions for where to go?

She will bring up everything eventually, so you don't have to...It depends on how comfortable she is with you when she will discuss...Be agreeable but not pushy...If she doesn't know you she may want to take things slowly...
