You asked about prices. It greatly depends on your location. It also depends on the length of time. Let's assume we are talking about one hour. The price ranges from $200 and up. It can reach $500 plus per hour in Manhattan. Are you expecting GFE/PSE? Greek is often extra. Is it an agency or an indy? Generally  speaking, you should decide if your comfortable with the rate quoted or advertised by the  provider. The rate is usually not negotiable. If she posts no rates, this is a sign that she might negotiate. Frankly, you don't get to decide what is "fair" as you put it. You get to decide if you're willing to pay the rate she is asking. If you don't think it's fair, find another provider.

I think your best bet is to find girls with at least 3-5 reviews (here on TER) that ALL score high, and you are happy with their price.

On price, I've rarely found a good one below 300 per hour. The good ones range from 350 to 500. At 350-400 in most cities (except NY) you can get a very pretty girl with a good attitude, good BBBJ (no CIM) and FS.

This is not a simple task for a newbie - the sheer amount of information available IS overwhelming but you have to bite the bullet and
1. Search your local area for providers on TER
2. READ each profile and a couple of the reviews
3.  Go to their websites to determine services offered and donation amounts.
With the above done, you can then determine the approximate market value for one hour of time with a lady - and remember, providers usually charge what they believe their time is worth, NOT necessarily what the market amount is in any one area...

As far as HER looks, well, that's what this site is for. Take some time to research the ladies. many hide their faces in photos but if you read the reviews you can often get an indication of what's in store. I don't know where you are but a "fair" price is a relative term.

Some care very much what you look like and the phrase, "You Mileage May Vary" has a lot to do with hygene and your appearance...

Price? There are exceptions to the rule, but you usually get what you pay for. You want a plowhorse or a thouroughbred?

Don't confuse neatness, being well kept and good hygiene with looks. I stand by my original statement. Would she rather be boinking George Clooney than Al Bundy? Probably, but with a true GFE, a professional provider, it's not going to effect the level of service you get.
