She will usually prompt you as long as she isn't suspicious of you.  She will usually ask "anything else I can do for you? or "anywhere else need massage".  I usually ask for "everything" if she asks me or use a hand motion for BJ or HJ (if she's not that cute).  BBBJ vs CBJ is usually her call, many only do CBJ, sometimes more after she knows you.  You can usually work out the details after you settle.
