in the registration process, you are asked to give the first name, year of birth. This information has to be true right? When you see a provider, you have to show your driver's license or passport (those that you submitted to p411 ) that has a name and date of birth matching p411 account info, Am I right?  

Do you scan and send your passport or driver's license?

Yes you send a scan of your drivers liscense.. You come up with a 4 digit number for something that yiou will have with you..When they asked me, I automatically used the last 4 of my SS #.. I then realized why they wanted it.. and contacted customer service whoo changed it to the last 4 digits of my direct dial # at work,, and I carry my business cards with me and that # is on them..  

You shoiw aDL when yiou meet the girl orsomething withthe # on it.. They do call you work # and leave a # codeand you have to then put that # into a web site so they can confirm your employment.. Pretty extensiive & I understand why.. so when I say to someone I am P411 screened.. That really should mean something if they understand the system

When I scan my ID and send to P411, can I cover other information except the four digit and first name?

Your ID upon arrival to the appointment. The number given to you at the beginning of your membership corresponds to documents provided to P411. P411 highly enforces the ID checking procedure because it has been seen that gentlemen share their membership with others. Providers can also report those members who either present a different ID or refuse to provide the ID altogether.  
Keep in mind also that P411 it is an added verification system to our usual procedures, providers can also request other pieces of information in order to complete the screening process.

If P411 wants you to submit a copy of your license or passport, they will let you know. IMO less is more for your own privacy and safety. I am not convinced P411 actually destroys all data like they say they do and from my view point, I do not want all that personal info of mine sitting on their servers and pc's. You will have to weigh the risks yourself.

I have never had a provider ask me for my license from P411 or otherwise. If someone ever does, I would just turn around and leave. If you are willing to share your license with a provider, at least make them ask for it, don't voluntarily show it to them

As others have noted, you do have to submit a copy of your license or other ID to P411.  And the info should be true.  If you're P411 verified, it's fairly rare for a provider to ask for ID, but they might --that's how the system works.  

One approach - before the appointment, place a piece of clear scotch tape over your license, and use a pencil or ballpoint pen to "black out" everything but your first name, birth date and last 4 digits of the ID number. That should be good enoughfor the provider and will protect your anonymity.
