Basically, there's an organ somewhere near the G spot that is analogous to the male prostrate, and it squirts a liquid that is fairly clear.  It is not urine, in any case.

The STD angle is something I've not heard discussed, but no one has brought is up as a problem, so I'm guessing it isn't a big deal.

If it is not proof of an orgasm, that I'd like to know what is.

As far as STD risks...I am sure they would be relatively the same whether the lady squirts or not. Vaginal fluids of any kind can carry an STD, if the lady is infected.

Performing oral sex is a risk, whether orgasm is reached or not. You just have to decide whether the risk is acceptable for you and your partner.
