A lot of contact forms ask for your name. Do you all provide your real names? I have somewhat of a unique name so I don't feel comfortable providing it, especially in my region. The only place that has my real name is p411. I have one provider OK on there but I feel like providers like to do more screening and want your real name. Any feedback on how to handle this is appreciated. Thanks!

It's simple really.  If you do NOT want to give your REAL name to a provider, then do not see a provider who Requires your Real name in order to see you. There are plenty of ladies out there that fall into that category.    (How would they know it anyways unless they require that you show them your ID when you arrive )   There are plenty of guys I know on p411 that have "hobby" names.   I do know that a p411 member is told that the provider may ask to see their ID to verify they are really the client..... If you contact p411, they can tell you how to do this and still keep other personal info on there private.

 I personally do not "care" what your real name is as long as you have references that "vouch" you are a safe,  financially responsible and respectful person to spend time with.  For anonymity reasons alone, I would not want a client to insist that they know MY real name prior to seeing me.  
A potential client seeing my reviews can tell them that I am a safe provider to spend time with.   :)
For anonymity purposes, Definitely come up with a hobby name.  :

Once I begin screening, I will know if your not giving your real name.  It's hard to screen someone who doesn't exist.  

And without real info, you may as well not screen at all because you've got no info without it.  

Sure I accept P411 and DC, but one of the absolute worst dates who behaved so badly I left him in MY hotel room because I couldn't wait to get away from him had 20+ highly recommendeds on DC.  So there you go.  I sure as hell wished I had gotten his info beyond DC so I could warn other ladies so they would not have had to go through what I did.

There's no reason for a provider to know my birth name, date of birth, etc.
I never filled out a contact form in my life.
I just email the lady and provide as much information as possible.
The references are most important.

The real name/employment info section are only if you have no provider references.
You're a member on p411 and you have one provider Okay, so you shouldn't have much trouble with getting dates.

Keep in mind that providers have different methods of screening.
There are tons of options out there, so never feel discouraged..

I don't just give out my full name to perfect "strangers." Typically, since I only see well established TER ladies, we have some PM exchanges and when I am comfortable that they are "discrete"  I send them my full screening info which consists of: DATE-Check ID, TER ID, 2 provider refs (their website & email) also a link to my website (with work phone, my cell phone, my picture, and my full name).
I tell them my approx. age but never date of birth or any other personal info.

I know that the most recent ladies I've seen did check my DC profile (shows up with time-stamp when they looked at it) but I'm pretty sure they didn't contact my refs. (they would usually let me know)

I would say that if you are going to be contacting a large number of providers, you might refrain from the real name.  But if you are just going to hobby once in awhile and particularly when just starting, the risk you have to take is giving your real name.   The reason is that if a provider is uncomfortable with you, it takes all the enjoyment out of it.  She will spend half her time worrying about you.  Once she is confident about who you are, the experience will be much better.  
Once you have one or two ladies who are comfortable and confident with you, they might even be able to connect you to others that she knows and you will have your own little network.    
Take your time and start with one or two well reviewed ladies and stick with them until you get a bit more comfortable with the game.
