What is best way to be sure all is safe - health and LE?

with multiple reviews (usually at least 4 or 5) by established hobbyists (guys with multiple reviews over a year or so).  There are no guarantees but you need to do what you can to minimize risks.

So, if someone has a number of reviews, can I be confident that is not an LE set-up?

If you are a hobbyist, this is simple:  See ladies that are well reviewed and regarded in the community.  From that, you should be be able discern that LE and health risks are minimal.  Just realize that nothing in life is 100% risk free, but if you use your head (the big one), you can play this game without being being penalized...

As we have seen recently in Chicago and Indianapolis, girls who don't screen well get arrested.  When they do, the cops take over their cell phone and sometimes their e-mail.
You need to find ladies who have strict screening and use safety precautions.
