1) Tip:  Optional, it really depends. You're already paying her a lot of money for a short period of time. If she blows you away, yes.  Otherwise, no.  When I first met my ATF, I was so taken by her that I paid for her room. But that's rare.  

2) Session overtime:  If it's an incall (her place) a gentleman will leave on time.  Don't make her ask you to leave. If you think you'll need extra time, book for 90 minutes.  She may not be able to give you more time at the last minute.   If it's your place and she stays longer without you asking, that's off-the-clock time.  Congratulations, she thinks you're a nice client.  

3) Multiple pops: I wouldn't discuss that over the phone, that's indiscreet & a lot of providers will hang up on you.  Check her website, P411 profile, reviews, etc.,regarding multiple pops.  Do your homework.  

4) Massage: go to an AMP.  

5) Screening: do yourself a favor: join a screening service like P411 or Datecheck.    --z

...These are all good questions but have been answered many times and the answers are all available by reading back a few pages on this board, reading the Newbie Manual and searching the topics you mention.  That said:
1) Tips are not required or even necessary.  Yes, they are appreciated, and different mongers have different views on this.  Do what makes you comfortable.  You will not suffer if you choose not to tip.  Sometimes I do and sometimes I don't.  Depends on the situation.
2) If the session goes over...First of all it's your responsibility not to turn the provider into a clock watcher.  You should be reasonably attentive to the clock and not presume on her time.  You have no idea if she has a session right after yours and it's none of your business.  She doesn't want to break the mood by mentioning the time. So let her know you're aware that your time is almost up.  She'll let you know what's possible.  She may choose to let you stay longer and not pay extra, or she may suggest you extend the session for a fee.  Up to her.
3) Multiple pop question over the phone.  A big no-no.  Most good providers will not discuss sex until you're in the door, donation left in plain sight and your clothes are off.  Otherwise it's too big an LE risk for her. If you metion any sex acts over the phone or in an e-mail, most providers will end the contact. This is why you need to read the reviews, which will tell you exactly what the provider will, and will not, do.
4) Massage.  I could care less but I suppose you can ask about that in advance because it's not an illegal act.
5) Screening questions.  Please just go to the web sites of various providers and you'll see what kind of info they want.  There is some variety but not a huge amount.

Seems to me a lot of dumb question around here that could be awnserd on your own.Ask your self the question first awnser it honestly if it sounds good go with it if it sounds bad don't do it. Some times trial and eror are key sometimes you have to make mistakes to learn from.I heard something before it went like this if it was imoral, unethical,or unsafe don't do it.This goes a long way in making decisions in all aspects of life ask yourself that when faced with a question.I do like the phrase there are no stupid questions only stupid awnsers but ask yourself first.whats next   Q1.how do I know I'm not LE?
                                                                                            Q2.should I start hobbying?
                                                                                            Q3.should I use twenties or hundreds?
I thought this was a sophistocated and respectable
venture thats the image I like to present part of
the fantasy to me. No disrespect to anyone...
