I've never heard it applied to gangbangs.

In the US, it sometimes refers to anal, but more often it is a somewhat vacuous asserting meaning A-One, or First Class.  It has no promise of any particular sexual practice.

I've been looking through AdultWork's search tool, I'm familiar with all but two of the services that are searchable... Those being
