Happens for variety of reasons such as Provider Retiring, LE issues, or she broke some TER rule inadvertently or inadvertently.

when I click on her name, a message comes up with "Providers information is no longer available on TER".  What does that mean.  Her name is still there on my favorites list, but you can't access reviews or stats.  What happened here?

could be one of several things.  In any event, she has been delisted.  If she retired, she may have requested it, if not, she must have violated TER policy and was delisted by admin.  In any case, she may reappear some day.
