Would someone please tell me how to remove an unapproved review from my list of reviews.

...your review list. You can fix whatever caused them to be unapproved and resubmit them if they're not too old. (must be within 90 days of the session).

I have an unapproved review because I reviewed the provider again about 2 years after my first review of her. When the new one was posted the old one became unapproved. It still shows up in my review list but isn't available for anyone else to see. There is no mechanism for me to remove it.

it replaces the original review and the original is labeled unapproved.  No one but you can see the unapproved review.  I sorta doubt admin will remove it as it is proof that you have reviewed the provider more than once.
