Pretty basic questions - provider asked me for city and state and phone info I used to establish my TER account. Is that legit? Can they access that to confirm? If so, where can I find that info since I cannot remember what info I used to set up my account?
and it's not even on your profile. You could tell her, but to me, it's a pretty useless and unusual request. Unless she has access to TER's servers, what could she find out? She may use it to check you out on other sites however, so be careful on what you tell her.

Maybe she meant "email." She can verify it is you by the email you gave TER.
All they require is a first and last name (which obviously many people fake) and an email address. Either you misunderstood her or she is misunderstanding TER. As BP said, she could check you from your email address, but nowhere does TER ask for a phone number (or city/state)
Maybe you and/or she are confused about the requested relationship between your name, phone number, city, state, and TER handle. BUT in my short experience, her or agency having all three is not unusual for safe screening.
