I don't understand how to create a profile with photos and then post provider ads. Is this even possible?

That is done by the first reviewer you have.  

And profiles do not have photos anyways. They do have links to your ads and website which should have photos.  

You can, however, place an ad with photos on your local board. I highly suggest that you read the Self Help Manual that is linked on this page which has the rules you must follow in order to place ads.
