Most providers list similar screening requirements on their websites.  

By the way, the hobbiest avoids LE and unstable providers by seeing women with lots of recent, positive reviews.

I have yet to take the big step to set up an appointment....It would help if I knew what information is wanted when you go through a "screening process"?

It varies from provider to provider.  Some will ask for references from other providers (which you don't have if you're new to the hobby), some will ask for your full name and work information, some will ask for a combination of these things.  A few will ask for all of the above.

The amount of information you give should always be consistent with your comfort level.  This sometimes presents a problem for newbies because the easiest, most 'anonymous' information to give is a reference.  

But stick with a well-reviewed provider and you can feel pretty secure about the information you give her.
