if a provider doesn't ask any screening questions is that a red flag?

She's well reviewed on TER (has over 100 reviews) and most of her reviewers have reviewed other providers in the area. Still makes me nervous that she didn't ask a single screening question. Could she be LE or working with LE?

But if she has a hundred reviews then maybe she verified you some other way or simply waiting til day of?  

ask guys who have seen her recently if she's still legit

There are many ways to screen that you don't even know it happened, and some ladies don't do much screening. With over a hundred reviews I wouldn't worry about LE but if something makes you feel uncomfortable follow your gut. With that being said don't let your nerves get the best of you or you'll never have a good time.

Do you have any reviews posted? Maybe she just looked you up, contacted one of the companions you reviewed and verified that you are legit. No further need for screening at that point.

Personally, I have a lot of experience interacting with people and "reading" them. Honestly, I've found that to be more informative than most screening methods. Additionally, a lot of info can be obtained from an email, online handle or phone number.
