I do apologize if this is a repeat question but I didn't pull anything up in the search. Is it common for angency to request this information? How safe is it for me to give my full name? What precautions should I take?

It's pretty common - in fact some will ask for your employment info as well in order to verify you. It's up to you to decide how much you're willing to divulge but do your homework first and check their reviews.

Note that if you go the Date-check or P411 route it's the same deal - they'll ask you quite a bit of personal info so that they can verify you. They don't release that info to the provider though unless you put it in your public profile where she's free to read it. If you're concerned about giving out your full name you may want to consider going the verification service route and that way you only really have to do it once. Be aware that as a newbie they may still ask for other info besides what comes from the service but they're less likely to do so.

I don't mean to sound like a cynical jerk but it seems to me that if this Tiger Woods thing has taught us anything its that we can only trust a provider or agency to be discreet for as long as it's in their best interest.  Don't let the fantasy suck you in.  They are in this for the money and if not "losing" your info may prove beneficial to them in the future assume they won't "lose" it.

I know that my blanket statement is not true for all providers but the assumption will probably make you safer in the long run.

Personally I'm working on creating a separate identity from my real ID and using that for all hobbying.  I won't let the two cross paths.  The down side is that I will have to assume additional risk before I can get a couple reviews and even then there will be providers I won't be able to see.

You take the good with the bad and you never know what tomorrow will bring.

My personal precautions include a prepaid cell that isn't linked to my real name, a prepaid credit card without my name on it (the universal gift card type), and an e-mail strictly for hobbying.  It's possible to go a lot farther if you're even more paranoid than me but that should be enough to keep you anonymous from all but LE assuming they could be bothered to look for the sake of us petty crooks.

But I admit to being a newb at hobbying and I've never used an agency.  So feel free to take or leave my 2cents on that basis.

I have never used an agency that didn't want my full name.  Most of the time the lady is going to want to see your ID when you arrive as well so don't try giving them a fake name.

Agencies are no different than indies.  Stick with agencies that have a good rep and well reviewed ladies.  Is it 100% safe? Well, no, but nothing in the hobby is.  We all have to decide what level of risk we are willing to undertake and accept the fact that our comfort level may limit the ladies that will see us.  It's just that simple.

In my past dealings hiring adult performers, I had to keep files with far more detailed info than that.

Remember, it's not a problem unless/until it is.

Any agency with a reputation to protect would be insane to send unknown women to the hotels/homes/ businesses of their clients without having the slightest recourse inre to their actions while acting as agents of the agency. The potential liability would be huge, not to mention the loss of their good name if the escort proved to be a thief, blackmailer, or worse.

It seemed more appropriate for a provider but questions like these are not uncommon from clients - this is the first one I've seen from a lady so I assumed it was a client with just an odd name.

Anyway, I've no experience with the management side of agencies but as a client I know the agencies I've dealt with know far more about their ladies than just their real names. I guess the safety level would depend upon the agency.

An agency would have good cause to know your (provider) legal given name...  & see your ID.  If you are underage, it could have serious legal consequences for them.  I would be glad that the agency strives to keep adult industry all adult.

In my opinion you should only disclose what you are comfortable with.  Agencies are different than independents though.  Maybe seek an independent with reviews that will verify you in a way you are comfortable.  Happy hunting.
