A very well reviewed provider (dozens of reviews--almost all of them giving her 9s and 10s for appearance and service) asked for me to email her a photograph of myself. I'm very interested in seeing her based on her reviews, the picture is a deal breaker. Is this common? Am I concerned about something I don't need to be concerned about? I think I'll just say that I am very interested in seeing her, but only if I don't need to send her a picture. Any thoughts?

Completely agree, it is your choice and if you are uncomfortable, see someone else.
I also understand why the request comes up: some guys are as bad about honestly stating their age, WEIGHT, or race as some ladies are about age and weight.
One tiny lady in Ohio clearly posted on her ad that obese guys would get a very limited menu. One guy claimed to be 200 but showed up at twice that and got very irate when she said only options were her on top. He got loud and demanded other variations and she said she was physically afraid of him (she may weigh 100 after an eating binge) so she relented, afraid something worse would happen. She suffered significant injuries just because he was a huge blubber mass. Since then I know she asks for a photo from new guys.
If its Beverly Lamour its up to you. I sent her one and she cancelled our date. I'm not ugly just 57 years old. Her level of service depends upon your looks. I say no way. Use the 100 dollar bill line. E-mail me for more info.
Once you send out your photo it will sit in her computer and will more then likely be shared. I don't care what she says, if another provider contacts her for a reference you photo will be shared. If she gets busted and her computer is seized, LE will also have it.
Don't let something like this come back and bite you in the ass later.
You could just be honest, tell her that it makes you uncomfortable but you'd really love to meet with her. Give her a little written description of yourself. If you agree to all her other screening policies, she may make an exception. If not then there are plenty of providers out there that do not require you to send a photo.
If she requires your full name to book, a photo doesn't seem like that big of a deal to me. Just send her a tiny photo from your linkedin or facebook. If anything happens then you can say "that is just a photo from linkedin..." You could even host it somewhere for free, send a url to it, and then delete it a few days later. It's unlikely she'd bother right click save as -ing you.
I have had people ask me for additional photos, ones which show my face. I decline when asked. That person is welcome to see a provider that makes their face visible in their ads, or someone who is willing to send additional photos. There are many fish in the sea! It's too bad because I have a nice face and it would increase my business.
