Second, do you have any reviews yet? If not on this site, perhaps somewhere else?
There are Ad Boards on this site and other review/message-board sites where you can post ads for free. Just be sure to read the rules/guidelines and ask any questions you have.
There's always BackPage, which I find to be an excellent ROI, but I'd be careful in HOW you advertise there. I'm including a link to one of my BP ads, to give you an idea of what I've found works for me, and if you're interested I can give you more specific info. 
Eros is in most major markets, as is Slixa, although I don't personally have any experience with the latter.
Cityvibe might work, depending on your location, as it is completely dead in some areas.

I am going to give you some HONEST advice in as nice a way as I can, so PLEASE don't take this the wrong way and to the BSU's that are sure to chime in, SHE ASKED for advice, I am not going out of my way to criticize her.
I honestly think the biggest problem right now is your rate. You have decent, but very limited reviews, Three of your reviewers have reviewed only you and the fourth is hardly a well known or respected reviewer either. You are asking for $500 hr which I am sure most people will agree is "top dollar" in the market you serve, by contrast Debra services the very same market as you, is by any definition very "well reviewed" with almost 150 reviews with average scores better than yours and charges 30% less than you.
I would never presume to tell a woman what she should charge for her services, but you did ask repeatedly what you can do to increase business. So I am going to as nicely as I can "suggest" you might at least consider at least a temporary reduction in rates at least until you gain a bigger following. If your phone was ringing off the hook I would never suggest you lower your rates, but by your own admission you are "pulling teeth" to get ten appointments a month. Wouldn't 20 appointments a month at say $400 hr be better than 10 at $500?
Since you're not yet very established, this shouldn't be too challenging.
If you are seeking to appeal to those who want a mistress-for-hire arrangment, then actively target those gentlemen!
Put that in your ads and on your website. Try advertising on RS-AVS and get listed on P411 too.
Get new, very professional photos which scream "UPSCALE" and exclusive.
You know.... evening gown, an expensive bottle of wine placed subtly in the background, high-end lingerie and pricey-looking shoes (red-bottoms optional lol) to all give the image you wish to portray.
I'm not terribly familiar with marketing to that specific an audience, though, so hopefully someone else can chime in with real advice for you.

not looking for a provider in an evening gown or going out of my way to buy an expensive bottle of wine to share with a hooker. I'll pass on mistress to hire or fake screams of upscale portrayed by pictures. However, if a hooker is charging high rates. She needs to have a flawless website/advertising and live up to expectations.

EDIT: Also, ML: Your ad says "Looking for Prefer411 reviewers. Date- check. Ter Reviewers in order to verify me." You should REMOVE D-C from that. D-C has been compromised and is no longer safe or useful.

I have everything but a website. Do those of you who have a website feel it is worth it to spend the money? It seems most ads link back to a website but the can say all the pertinent info. Seems like a maze that needs to be navigated not once but twice. Thoughts....
There are even free websites you can get up and running in a matter of minutes.
I had one for the better part of a year, when I first returned to the business, and it was as cookie-cutter as you can get yet it still helped!
For some men, having a personal website lends credibility and also shows she is taking this endeavor seriously. Many ladies at the price-point you want to be have custom websites.
