If you're her first client of the day then she may charge more for you to see her to make up for the price of the motel room. If you're the second or later client then she'll charge you her Incall rate. If you're a regular client then she may waive the price of the motel room on you.
If the out call and in call are the same rate then it's really an out or in rate, It's just the rate.
As for the room I would assume that's simply treated as overhead and built into the pricing structure so you can be sure you're paying for it (or part of it). Is your question why wasn't there a different rate for out call versus in call? Or why we're you explicitly charged something for the room?

For me when I'm home based in the Raleigh NC outcall is much preferred for me. While I don't charge more for incall when I'm in my local area, I do require a booking of at least 2 hours. My costs for outcall is the gas in my car that's it. No taxi needed.
If "she" gets the room, it is by definition an "incall" regardless of whether she sees one client of a dozen that day in that room. Incall, you go to her, Outcall, she comes to you. Come on folks, if you are going to give advice here, at least know the basics.
