You do not need to tip at all. Providers do not expect it nor will they think badly of you if you do not tip.
Generally, I do not tip independents, but if I see an agency girl who I really enjoy I consider a tip, since she is only getting a fraction of the fee. The tip would be in the 10-20% range, but you can really leave whatever you want.
The best tip is to reschedule another visit. This gives the provider the knowledge she will have a repeat customer (more revenue coming) plus she gets the knowledge you liked what she provided and want more.
I never tip unless it's something very special, like her birthday or some other significant event.

You THANK HER first of all for her wonderful services, you tell her youll definetly see her again..and right as she heads for the door tell her OH wait let me give you your tip! She'll smile and walk towards you, simply lean forward & whisper this in her ear..."don't walk alone late at night, u might get mugged"
Personally I vary my tip from 0 to about 20% based on how great the experience was or was not, and I tend to tip more to agency girls who I know are not getting that much of the donation anyway. If an indy I also vary based on how much her rate differs from the norm.
As in any other walk of life, regardless of what some might say, you can bet your ass the ladies always remember who tips or brings gifts and who does not. Let common sense be your guide.

No tip. Good providers shouldn't really be expecting it as their rates are higher BECAUSE of the quality of service. Set up another appointment and mention your good experience here on the board and the return for her should be worth more in the long run than the $ you might give her as tip. I'd say the best, if not only, reason to tip would be if your appointment went outside of the agreed upon parameters. She provided wine/toys/service you weren't expecting or if your time ran long and there was no mention of it. If it is just a matter of you having a really good time, that is just part of being with a good provider...which if you did your research here you should have been expecting and paid appropriately for up front.
