how do i get a providers id number if the provider has no review??

...writing a review and filling out the profile just leave the ID# field blank. It has an asterisk by it which means not required. I would state in the general details section what her provider name is and say this is her first TER review. I am of course assuming you are trying to review a new provider. If you aren't then just know there is no TER ID # until she has a review.

her profile will be created by what you indicate in the review template page.  Once your review is posted, she will be assigned an ID# by admin.  As xyx said, just leave her number blank when you fill in the review form.
