Among the girls, is the distinction between "massage" (or "rubdown") and "escort" a useful one when it gets right down to what they are willing to do with you for the one hour you paid for?  Are there any code word or phrases that girls use in their ads to subtly communicate how far they will go?  Is it a good guess that the girls in the $300 range will go all the way while the girls in the $200 range will use nothing more than her hands?

Look for the term "full service".  If they provide full service, then expect to be able to have vaginal and oral sex with them.

The 200 and 300 distinction is a good indicator. I have found that massage-only girls are pretty careful to say "no sex" or "no fs" in their ads. They don't want to deal with explaining it to every customer they see. Some escorts who advertise in the massage sections, want to be more selective about who they offer FS to, and they decide on that when the client arrives, after they've spent a few minutes with them.

At that point they may let you know that they'll accept 200 or 300 depending on what level of service you'd like. I would expect them to be very discrete about this, and if they say the word "sex" - run like hell. (before the guys with the cuffs come in)

200 to 300 is usually a good indication, but it does depend on what part of the country and which town you are in. I'm from San Diego and 200 to 300 is a good indicator. Where as in LA you can get providers that will only do massage for 300. Looks is another indicatior, if she looks like she just walked off the cover of magazine, more than likely 300 is not going to get you much (unless she is Russian). Also if she offers you a massage for 200 and you throw in more cash for more options, that's YMMV.Hope this is not rambling on too much.

Whatever you do, DON'T ask a lady about what they will or will not do. DON'T even ask using code words like "full service" or "complete package". Even if a lady does provide this, they will probably say no and hang up.

If a provider is ONLY listed in a Massage category in a web directory, then that's pretty much a guarantee that there is no penetration. Escort USUALLY means penetration - but as always Your Mileage May Vary (YMMV).

Bottom line - trust the reviews. When you see the lady, let nature take it's course :) Let things heat up before you start talking about sex.

I realize this could be a problem if you are in an area of the country that doesn't have a lot of choices.

I don't think most providers advertise the term "full service" anymore. AFAIK, LE knows what that means and it's enough to get busted.

Speaking from an L.A. perspective, I agree that the 200 and 300 price range is a good indicator of what you can expect.  200 is probably full body sensual massage of some sort.  But, as you can see from some recent reviews, some newer or less greedy girls will go all the way for 200.  For me 300 is an assumption price.  I assume that I'm getting full service unless I'm told otherwise.  

I have in the past seen girls for a massage and afterwards was told of other options which upon further visits I indulged in.  Once I called girl in the massage section of a website and was given a subtle breakdown on the phone.  It was along the lines of "for $200 I offer a full body massage.  For $300 I offer something more intimate."  Right there that's a big indicator that she offers full service.  Just listen carefully and don't say much.
