You send a provider an appointment request through email or 411. Couple days go by and you don't hear back so you book someone else. Then you hear from the first lady. Ok to just say sorry I made other arrangements?

.....I would nicely tell lady number one, "that when I did not hear back from you within a few days, I made other arrangements".  

  I would fully expect a gentleman to make other arrangements, if he did not hear from me within 24 hours.

You did your part.  However, the downside is, you don't know why she was slow responding to you original request.  But I do feel  that you should let her know why you are not going to see her.  Who knows, you might want to try to see her in the future, and you don't know how she will react to your silence if you don't respond.
